Tournament Agreement:
I have reviewed and agree to attend all tournaments as "accepted".

Practice Agreement:
I understand that I am obligated to attend practices.  If I cannot attend, I agree that I must make contact with the
coach and that I must receive a return reply if contact is by email or text.  If I do not get a return reply within one
hour of practice time, I must call Trina Sharpe @ 704.450.1718.

Terms:  
  • Deposit (submitted with your ACCEPTED registration).  50% of balance due on receipt.  Balance in full due
    December 15 2011.  I submit that I am obligating myself to this schedule and agree to make payments as
    stated
  • I agree to pay tournament per diem each tournament and coaches hotel fees if required prior to the first
    match of the day.  Payable in cash to team rep

I understand no refund is applicable if:
  • I decline a Carolina Region Tournament
  • I withdraw from a tournament for any reason (including injury or illness)
  • I remove myself from the team or cannot continue the season for any reason
  • In case of serious injury that prohibits me from continuing the season, I may appeal
    and request a credits.  Request will be investigated and evaluated, however credits
    are not guaranteed
  • The team withdraws from a tournament after the deadline to receive a refund
  • The team enters a non Carolina Region Tournament and withdraws
INSTRUCTIONS
Please review the proposed tournament schedule below.  Make your commitment by clicking
the ACCEPT button for each tournament.  If you decline a tournament, please explain in the
comment section below.

Fee:   $795

Your fees include:  
  • (4) Carolina Region Tournaments or Equivalent
  • Quest for Excellence Tournament
  • Regional Championships

Your fees do not cover:
  • Optional Tournaments
  • Jersey Deposit
  • Shorts
  • Travel:  Personal and coaches hotel over 75 miles one way
  • Coaches per diem
  • CRVB tournaments:  $5/day
  • Out of region Tournaments $25/day
  • Performance training and any practice over 2 per week
  • $25 ZX Fitness Processing Fee
  • (Platinum Preferred Members Fee)
